Please take a moment to read all of the following statements.
I understand that massage therapy given here is for the purpose of stress reduction, relief from muscular tension and spasm, general relaxation and/or improvement of circulation and energy flow.
If I experience pain or discomfort during the session, I will immediately inform my therapist so that pressure/strokes can be adjusted to my level of comfort. I will not hold my therapist responsible for any pain or discomfort I experience during or after the session.
I understand that the services offered today, and in the future are not a substitute for medical care. I understand that my therapist is not qualified to perform spinal or skeletal manipulations, diagnose, prescribe, or treat physical or mental illness.
Because the massage therapist must be made aware of existing physical conditions, I have stated all my known medical conditions and injuries and take upon myself the responsibility to keep the massage therapist updated on any changes in my physical health and I understand that my massage therapist and Spabulous Massage and Spa shall not be liable should I fail to do so.
I understand that massage offered is strictly non-sexual in nature and any illicit or sexually suggestive remarks or advances will result in immediate termination of the session.
I hereby waive and release my therapist, Spabulous Massage and Spa and its staff, massage therapists and spa specialists from any and all liability, past, present, and future relating to massage therapy.
I have received the policy statement, and have read and agree to the policies therein.
